Updated Early Help Assessment Form

An updated Early Help Assessment Form is now live on the NYSCP website.

The update includes the children&families@northyorks.gov.uk email address within the form and changes to the consent section removing the requirement for referrers to complete a separate Children & Families Service consent form.

This changes ensures the Early Help Assessment Form is consistent with the Universal Referral From in how consent is obtained.

The form can be accessed below and from the Early Help Page, Worried about a Child Page and Home Page.

All previous versions of the form should be discarded.
